CONTACTS TAB - Client Profile
These fields give you the place to document information about those persons that may and/or must be contacted about the client under various circumstances.
There are individuals that you may be legally required to contact about the client in certain situations, so it is important to document all Contacts correctly.
“Contacts” is a general term that may include the client’s family, friends, agency workers, service providers and/or other people important to the client.
How to Document a Contact
Contact Person
Relationship to Client:
- Select from the drop-down list the Contact Person’s relationship to the client
- Pick only one of the following answer choices:
- Child
- Friend
- Grandparent
- Parent
- Partner
- Sibling
- Spouse
- Other
First Name (Required for mental health and substance abuse)
- TYPE the Contact’s first name into the textbox.
MI (Middle Initial) (Not Required)
- TYPE the Contact’s middle initial, if known, into the textbox.
- If unknown or the Contact does not have a middle initial, leave blank.
Last Name (Required for mental health and substance abuse)
- TYPE IN the last name of the contact person.
Legally Authorized Representative (Required for mental health and substance abuse)
- If the contact is the Legally Authorized Representative for the client, indicate the type.
- CLICK ON the arrow
to the right of the drop down box to view the answer choices.
- The answer choices are:
- No
- Parent Guardian
- Legal Guardian
- Guardian ad litem
- Managing Conservator
- Other
Emergency Contact
- If the contact person is the “designated” Emergency Contact for the client, CLICK IN the box to make a checkmark.
- “Designated” means either designated by law, by the client or by organizational policy, whichever takes precedent. In some situations, authorization from the client may be required for contact.
Designated Follow-up (Required for Substance Abuse Services Only)
- If the contact person is the “designated” post-discharge Follow-up Contact for the client, CLICK IN the box to make a checkmark.
- “Designated” means either designated by law, by the client or by organizational policy, whichever takes precedent. In some situations, authorization from the client may be required for contact.
Primary Contact
- If the contact person is the “designated” Primary Contact for the client, CLICK IN the box to make a checkmark.
- “Designated” means either designated by law, by the client or by organizational policy, whichever takes precedent. In some situations, authorization from the client may be required for contact.
